How to apply

Paid Family Leave claim to care for a family member

File Part A online in your own myEDD account, or on a paper form you order from EDD. Part C always goes by mail.

Getting help

You start this yourself

You, the worker, file and sign your own claim. If you cannot act for yourself, a legal representative can call EDD to ask how to file. Your family member signs their part, Part C. If they cannot, someone with power of attorney or a court order can sign. A spouse or domestic partner who wants to sign for them must contact EDD first. Their doctor fills out Part D.

Only you, or someone with legal authority for you, can sign.

What happens after you apply

  1. Order the paper form from EDD. It comes in about 10 days. Use paper if you have no California license or ID card, or no Social Security number. You must also use paper if you recently changed your name.

To keep it
Pay lasts up to 8 weeks in any 12 months. Leave you used in the past 12 months counts. Report wages or paid time off. If others share the care, you are paid only for time you are the main caregiver. This program pays you but does not protect your job. Another state law may.
